12 estates of HKHS, namely, Healthy Village, Kwun Tong Garden Estate, Prosperous Garden, Cho Yiu Chuen, Kwun Lung Lau, Ka Wai Chuen, Clague Garden Estate, Jat Min Chuen, Ming Wah Dai Ha, Lok Man Sun Chuen, Moon Lok Dai Ha, Bo Shek Mansion

How it works

Multiple co-morbidities are common among elders, and poor drug compliance limits the functional state of the elders and lowers their quality of life. This programme provides health assessment and personalised health education/advice to the elders and their caregivers so as to improve their dietary habits and lifestyle.

A nursing professional is based part-time at the housing estates, providing health care consultation to 500 elders with multiple co-morbidities and polypharmacy by using the Health Case Management approach.

Project components

  • Individual comprehensive health and medication assessment;
  • Individualised health care advice /consultation;
  • Medication compliance monitoring;
  • Lifestyle/dietary advice, health practice demonstration and education.


  • To improve health conditions of elders with multiple co-morbidities
  • To train up elders and their caregivers in medication compliance and disease management
  • To enable elders and their caregivers to build up healthy lifestyle habits and practices
