Elders living in the remote Tai Po rural areas are isolated and have difficulty maintaining healthcare and social services. The elders always miss medical consultation and affect the treatment progress. They also lack cognitive and educational activities which are helpful for their quality of life. The project will set up rural Holistic Health Hub services in village offices (村公所) in Tai Po and Sai Kung North so as to provide elders with more medical and social support.

Project components

• 720 basic health screening services
• 216 elders will receive medical assessment
• 360 elders will receive nursing consultation
• 360 elders will receive exercise training
• 360 elders/carers will join health programmes
• 288 elders with early dementia will join cognitive training
• 50 community supporters will receive voluntary service training
