Multiple co-morbidities are common among elders, and poor drug compliance limits the functional state of the elders and lowers their quality of life. This programme provides health assessment and personalised health education/advice to the elders and their caregivers.
In order to improve their dietary habits and healthy lifestyle, a nursing professional is based part-time at the housing estates, providing health care consultation to 500 elders with multiple co-morbidities and polypharmacy by using the Health Case Management approach.
