Project Period

Jan 2019 to Dec 2020

Target Group

Elders with early dementia, frail elders, their carers and the community of Wah Fu Estate and Wah Kwai Estate

Location

Wah Fu Estate & Wah Kwai Estate, Hong Kong Island

Wah Fu Estate is one of the oldest public housing estates in Hong Kong. With the growing number of vulnerable elderly residents and their caregivers, the Caregiver Companion Project (Phase I) was implemented in 2016-2018 and a dementia-friendly community service model was piloted to support the persons and families with dementia. The dementia-friendly community service model will be extended to Wah Kwai Estate, a district next to Wah Fu and with 21.1% of the population being aged 65 or above.

Project Component

  • 100 frail elders will receive day care support
  • 70 early dementia elders will join the talent hub
  • 50 elders will join Saturday respite
  • 100 caregivers will receive training and respite service
  • 30 volunteers will be trained as assistants
  • 15 key stakeholders, e.g. residents associations, security guards, shop-keepers etc, will be connected and engaged in dementia-friendly community activities
  • 250 people/neighbours will participate in the Smart Café community programme
  • 500 people/neighbours in the community will be trained in dementia awareness programmes

Impact

The frail elders and those with early dementia and their family members will make use of day care and community support services to frail elders in enhancing their quality of life and relieving pressure on carers. The potentials and talents of elders with mild cognitive impairment will be maximised at the Talent Hub. In addition to Wah Fu Estate, the project will also engage key stakeholders at Wah Kwai Estate to build up a dementia-friendly community.

  • Demonstrate a one-stop integrative community service mechanism for frail elders and those with dementia
  • Develop a talent hub for early dementia / mild cognitive impairment elders
  • Tailor-made home-based cognitive training
  • Empower and support caregivers, particularly adult children caregivers
  • Empower senior volunteers as enablers/activity assistants
  • Line up strategic community partners from various disciplines
